For remote patient monitoring programs, Stream Health identifies eligible patients based on diagnosis codes (diabetes, hypertension, CHF, COPD, etc.), generates patient enrollment lists, tracks device-collected data (blood pressure, glucose, weight, pulse oximetry), and creates billable encounters automatically documented in eCW with appropriate CPT codes and supporting clinical notes.

Consolidated patient summaries, care gap alerts, and RPM encounter documentation are delivered back to eCW via API write-back, appearing in the patient chart, clinical inbox, and health maintenance tracking modules.
