StreamForma is committed to ensuring digital accessibility for people with disabilities. We are continually improving the user experience for everyone and applying relevant accessibility standards. Our Goal: Make streamhealth.co accessible to the widest possible audience, regardless of technology or ability.
We strive to conform to the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA developed by the World Wide Web Consortium (W3C). WCAG 2.1 Principles: Perceivable (Information and user interface components must be presentable to users in ways they can perceive), Operable (User interface components and navigation must be operable), Understandable (Information and operation of user interface must be understandable), Robust (Content must be robust enough to be interpreted reliably by assistive technologies). We aim to comply with Section 508 of the Rehabilitation Act (29 U.S.C. 794d), which requires federal agencies and entities receiving federal funding to make electronic information accessible.

Forms and Interactions: Form Labels (clear labels for all form fields), Error Identification (errors identified and described clearly), Error Suggestions (suggestions provided to correct errors), Required Field Indicators (required fields marked with asterisk and text), Time Limits (adequate time to complete forms). Assistive Technology Compatibility: We test compatibility with Screen Readers (JAWS, NVDA, VoiceOver), Browser Zoom (200% zoom without horizontal scrolling), Speech Recognition Software (Dragon NaturallySpeaking), Keyboard-Only Navigation (no mouse required).
Some third-party content integrated into our Site may not be fully accessible: Calendly (appointment scheduling), Typeform/Jotform (form submissions), Embedded videos from YouTube or Vimeo, Social media widgets (LinkedIn, Twitter). 6. Accessibility Testing and Evaluation: Internal Testing - We conduct Quarterly accessibility audits using automated tools: WAVE (Web Accessibility Evaluation Tool), axe DevTools, Lighthouse (Google Chrome). Manual testing with assistive technologies: Screen readers (JAWS, NVDA, VoiceOver), Keyboard-only navigation, Browser zoom (up to 200%). External Audits: We engage Annual third-party accessibility audits by certified WCAG consultants, User testing with individuals with disabilities, Remediation of identified issues within defined timelines.
Encryption: Data in transit (TLS 1.2 or higher), Data at rest (AES-256 encryption), Email encryption for sensitive communications. Access Controls: Multi-factor authentication (MFA) for system access, Strong password requirements, Automatic logout after inactivity, Audit logging of access and activities. Transmission Security: Secure file transfer protocols (SFTP, HTTPS), Virtual Private Networks (VPNs) for remote access, Secure messaging platforms. Integrity Controls: Data validation and error checking, Version control for documents, Checksums and digital signatures to detect tampering. Audit Controls: Logging of all access to systems and data, Regular review of audit logs, Automated alerts for suspicious activity.
ADA Compliance: Stream Health is committed to compliance with the Americans with Disabilities Act (ADA) and ensuring equal access to our services. ADA Title III: Prohibits discrimination on the basis of disability in places of public accommodation (including websites). Section 508 Compliance: We aim to meet Section 508 of the Rehabilitation Act standards for electronic and information technology accessibility. These Terms and any disputes arising from or relating to the Site or Services are governed by: The laws of the State of California, Federal laws of the United States, Without regard to conflict of law principles. For claims not subject to arbitration, you agree that: Exclusive jurisdiction lies with the state and federal courts located in Orange County, California, You consent to personal jurisdiction in these courts, You waive any objection to venue in these courts.
